Admissions Steps
If you would like to schedule a tour, please contact the office @ 713-943-9978 or come by Mon – Fri from 9:00 am – 4:00 pm.
All secondary students (6th – 12th grade) are required to meet with the administration as part of the application and acceptance procedure. Please call the office to set up a time when you can come with your student. Please bring your child’s last report card with a conduct report and a high school transcript for all high school level grades attended to date (if applicable). If you’re ready to enroll your child, please fill out the enrollment forms and email everything to jparker.faithchristian@yahoo.com or you can bring it to the office.
- If your child is going into PK: fill out the PK4 enrollment form
- If your child is a returning student: fill out the Reenrollment form
- If you’re a new student: fill out the Enrollment form
- All students need to fill out the Student Emergency Contact & Info form (front and back) (If you have more than 1 student, you can put them on the same form)
- FCA does accept some students with a 504 or Accommodations documentation on a case-by-case basis. For more information, please contact the office to inquire.
Enrollment Resources
Please complete the form that applies to your situation:
- Returning from last school year: Complete the Reenrollment Form.
- Returning after being away for one or more years: Complete the Enrollment Form.
- New to the school: Complete the Enrollment Form.
- Enrolling multiple students: Submit a separate form for each student.
Enrollment Forms
Required Documents
- Child’s Birth Certificate
- Child’s Social Security Card
- Child’s Immunization Record
(exemptions accepted) - Copy of parents’ IDs
- Copy of last report card (PK4 and Kinder not applicable)
- High School Transcript (for all high school grade levels attended to date)
- A recent hearing and vision screen. (See Pinewood Screening for more info)
- 504 or Accommodations documentation (if applicable)
- Court/Custody Documents (if applicable)
International Students
FCA is approved by SEVIS to accept international students and to issue I-20s.
